This form is a request of cancellation. Your membership will NOT be cancelled immediately. Your membership will be cancelled once it is reviewed by our membership department. You will receive an email once your membership has been cancelled. Any request to cancel a membership before the end of the 12 month commitment is subject to the cancelation fee equal to up to three months of membership cost. Thank you.
